First time application:
- Matriculations (yellow card) for at least one vehicle and two motorcycles owned by the company
- List of personal protective equipment in possession
- Health insurance of staffs
- Technical information about the water source, treatment plants, reservoirs, water distribution systems, pumping stations
- Schematic diagram information of the water treatment plant(s) & map of the distribution network (if any)
- Detailed description of the water quality plan including: laboratory, equipment, technology and staff
- Detailed description of the water quality plan including: Sampling procedures and techniques
- Detailed description of the water quality plan including: Types of tests to be carried out
- Policy to protect the water resource
- A 5 years business plan
- Detailed information on wages, salaries and other fixed operational expenses
- RDB Certificate of company Registration or Copy of certified statute in case of a cooperative issued RCA
- Health and safety policy
- Customer/Client Service Charter
- Copies of all relevant contracts, agreements, permits or Memorandum of understanding (if any) with stakeholders you work or intend to work with
- Service level agreement(if applicable)
- Tax clearance certified issued by RRA
- Clearance certificate issued by RSSB
- Detailed description of water quality plan
